Custom Bracelet Design Form

Thank you for your custom bracelet order!

Now we just need to capture the word and colors you want in your design.  

Here's how this works:

  1. Fill out the form below and submit it to us.
  2. We verify the information and send any follow up questions or answers.
  3. Otherwise, if everything's in good shape, we add your necklace to our custom design queue.
  4. Your custom bracelet will ship approximately 7 days after receiving your design details.

If you ordered more than one custom bracelet, please fill out a form for each unique design. If you're using the same design for each bracelet, just note that in the comments section.
