Custom Necklace Design Form

Thank you for your custom necklace order!

Now we just need to capture the word and colors you want in your design.  

Here's how this works:

  1. Fill out the form below and submit it to us.
  2. We verify the information and send any follow up questions or answers.
  3. Otherwise, if everything's in good shape, we add your necklace to our custom design queue.
  4. Your custom necklace will ship approximately 7 days after receiving your design details.

If you ordered more than one custom necklace, please fill out a form for each unique design. If you're using the same design for each necklace, just note that in the comments section.
